In accordance with the Affordable Care Act, your employer within TruStage Group has been required to report certain medical coverage information to both the IRS and you. As part of this requirement, if you were in a TruStage Group self-insured plan (currently Anthem) since 2015, you would have received a Form 1095-C from your employer.
The federal requirement to send a Form 1095-C has changed so that employers are no longer required to automatically send a copy of Form 1095-C to employees if employees have been informed of their ability to request a copy of the Form 1095-C.
Based on this change in the law, we have determined that we will no longer automatically send a Form 1095-C, except as may be otherwise required by state laws. However, you may request a copy of your Form 1095-C at no charge as described below.
